Vancomycin IV 750mg Answer
by: Anonymous

You would draw up 15ml from each 20ml vial for each IV dose. The ration is 750mg/1000mg, which is the same as 3/4, as well as 15ml/20ml. You use the whole 1gm vial drawn up in the 20ml vial, but only use 15ml of the mixture to inject/compound into the D5W 250ML bag for 1 dose.
