Both Provider and Pharmacy decide the days supply

Generally, both the pharmacist and the doctor will decide how many days supply it is.

Oftentimes the provider/doctor/PA/Nurse practitioner will write on the paper prescription or on the escript how many days the prescription is intended to last.

So the answer is both the doctor and the pharmacist or pharmacy technician.

The pharmacy must follow what the provider has written on the rx, so that information trumps anything the pharmacy would decide. Also, certain medications have certain dosing schedules which must be followed (generally the manufacturer of the medication (the pharmaceutical company that made the drug) has figured out how the medication is intended to be used. So consulting the drug company's package insert would tell you how they intend for it to be used, and the maximum daily dosage is.

Unfortunately, I cannot speak to how often or when you can get your Xanax filled again. Please contact your provider or the pharmacy for the answer to that question.
