I have an order for Coumadin 7.5 mg po qd and I have a supply of Coumadin 5mg tablets. What is the dosage??

The dosage is 1 & 1/2 of the 5mg tablets by mouth daily.

Coumadin (also known generically as warfarin) is an anti-coagulant. It works to prevent clots, and therefore, reduces a persons risk of stroke, heart attack, pulmonary embolism, and venous thrombosis. Coumadin therapy requires patients to get their blood tested regularly to monitor their levels. This blood test is called an INR test, also known as a Prothombin time test, and it tests how long it takes the patients blood to clot.

If you are trying to achieve a dosage of 7.5mg a day with 5mg, then you will use 1 1/2 tablet everyday. So for a 30 day supply you will need to dispense a quantity of 45.
