Dosage Form - Billing Codes

Billing Codes for Compounded & Non-compound dosage forms
This is a two part question. Do I need to include the dosage form on all pharmacy claims or just those billing a compound drug? If on both compound and non-compound are the billing codes different?
Can you provide the current valid value codes?
